Privacy Policy

At Physio F/X ltd. all clinicians and administrative staff are bound by law and ethics to safeguard your privacy and confidentiality of your personal information.

The guidelines for protection of personal information are governed by PHIPPA (Personal Health Information Privacy Policy Act). These guidelines require us to ensure your personal information is utilized and disclosed responsibly, and only as necessary for the purpose we identify to you.

This privacy policy applies to any collection, use or disclosure of personal information and clinician treatment notes. In order to provide treatment it is very important for us to collect personal information including the client’s health history and previous surgeries that they have received. The information collected helps our clinicians determine the best course of treatment, monitor progress and change treatment if necessary. Without a signed direction of release of confidential medical information form we are not allowed to disclose any information to other practitioners or any other individuals.

We are legally responsible for all the personal information that we have. We are required to protect it against unauthorized disclosure, use or modification. The personal information is retained for 10 years after the client’s last visit and/or 10 years after the client’s 18th birthday.

Covid Policy

At Physio FX, our strict infection control proceedures have been in place since 2004. Once COVID-19 became a global pandemic our processes were further enhanced with the addition of mandatory face masking for all team members and clientele in the addition to social distancing measures. Hand washing, equipment cleaning, the use of individual electrodes, and up to date understanding of infection control measures will continue to be an essential component of our daily routine.

Clients who arrive for their appointment demonstrating signs of illness (COVID-19 or any other transmittable pathogen) will be asked to reschedule their appointment to limit the possibility of spreading. If a person in your immediate circle of contact (either through work, home, sport or social) have displayed signs of illness and there is a possibility that you may have been exposed to a contageous pathogen, we respectfully request that you attend your appointment wearing a mask and advise your clinician.

Clients who are immune compromised or who may reside with individuals who are compromised are encouraged to advise their treating practitioner and we will happily wear a mask.

Informed Consent

Our team of health professionals adhere to the highest standards of care while creating a welcoming and safe environment for EVERYONE.

Your therapist will gather some general health history and specific information from you regarding your concerns in order to determine the most appropriate course of treatment.

Assessment and treatment interventions will be discussed with you by your therapist(s). Our duty is to inform clients of material risks associated with treatment interventions, and anticipated outcomes.

We will not proceed without your expressed consent.

Your participation in your care is paramount to overall success. Our team members will request your consent with each intervention.

Please note you can withdraw your consent at any time for any reason.

Cancellation Policy

We reserve appointments with your therapist specifically for you. If you cannot attend your appointment we would appreciate if we could have enough time to fill the vacancy with another client wishing to schedule an appointment.

Therefore, cancellations must be made 24 hours prior to your scheduled appointment. Any missed appointment or cancellation with LESS than 24 hours notice is subject to a charge of :

Please note cancellation fees are not covered by your insurance, and cannot be submitted for reimbersement.

Fees are expected to be paid prior to your next service.

We recognize that emergencies, illnesses and poor weather conditions can arise without warning. Thus, each situation will be individually reviewed and fees could be waived on a case by case basis.

Repetitive missed and/or cancelled appointments will be discussed with individuals prior to applying the cancellation fee(s)

Payments Policy

ALL CLIENTS of Physio FX MUST acknowledge that fees incurred for products and/or services are ultimately their responsibility.

Fees at the initial visit are to be paid once a service has been rendered regardless of third-party payment.

Our team will endevour to process third-party transactions on your first visit; however, in many cases the payments cannot be directed to pay the clinic directly or the service may only be partially covered.

Therefore, we request each patient to come prepared to pay with VISA, MasterCard, DEBIT card or CASH for the services in case complications arrise with third-party payment systems.

You will be requested to sign a payment agreement policy at your first visit as affirmation of your agreement.
